What to Expect on Your First Visit

About us:
Smile Family Dentist has been serving this community since 1989.
In the beginning it was part of a comprehensive clinic and then branched
out on its own and has been located at 247 E116st since January 2001.
This central location of the “Barrio” is essential for
allowing us to continue to serve the people of the neighborhood, who
have been coming to this office for years, and allows us to grow, and
see patients who travel ad don’t live close by.
Our range of services has grown to include not only essential services
like fillings, and cleanings but also a wide range of cosmetic services
like bonding, laminates and orthodontics.
We are accessible by uptown bus, downtown bus, and close to #6 train.

Monday: 8am-5pm
Tuesday: 8am-5pm
Wednesday: 11am-7pm
Thursday: 11am-5pm
Friday: 9am-4pm
Closed Saturday and Sunday

3rd Avenues

What to Expect on Your First Visit
Payments
When you arrive for your appointment you
will be asked about your payment method, these may include insurances,
Medicaid, and private.
Private patients don’t have insurance, so they are paying.
Some methods of paying include checks, money orders, credit cards,
and of course cash. We also have office financing.
We accept most insurance for exams and basics services. Sometimes
there is a co-payment for services such as bridges, crowns or dentures.
Health History
When you arrive, you will also be
asked to fill out a health history form. This allows us to see what
procedures or anesthetics are appropriate for you.
We use the latest and best sterilization techniques in the office.
All patients are welcomed to see our techniques. No patients are discriminated
against by what is in their health history.
Personal Information
You will be asked questions
such as name and address.
All information will be kept confidential. This information will
be used for billing purposes and recalls which are cards sent every
six month to remind the patient for a new check up.
